COMMUNITY PSYCHIATRIC CARE TEAM HOSPITAL USM
 
  • Conduct home visits for patients who are identified during community meetings
  • Provide medication for psychiatric patients who are unable to come to the hospital for reasons that are reasonable and inevitable
  • Provide psycoeducation to families / caregivers
  • Refer patients to government agencies and non-governmental organizations if necessary
 
MISSION 
 
A team of professionals from various disciplines that are dynamic and progressive. This team is committed to improving the ability of individuals with mental illness, their families and the surrounding communities. Provided direct care in the community patients through the use and allocation of resources to improve the cooperation of nurses and family aimed at reducing the cost of treatment and reduce other side effects.
 
VISION
 
Movers in terms of promoting mental health and optimal biopsychosocial rehabilitation for individuals suffering from this mental illnes achieved through self-development, members of the community and all those involved with mental health needs in the community.
 
OBJECTIVES 
  • Providing multidisciplinary care and rehabilitation directly in the community for individuals with specific mental illness and their families and communities.
  • Ensure continuity and consistency between the mental health care community and the hospital.
  • Identify early signs of relapse and provide immediate treatment to reduce the need and dependency on admission to psychiatric wards
  • Promote recovery (recovery) function and quality of life with appropriate rehabilitation program.
  • Facilitate and enhance the participation of all parties with mental health services.
  • Ensure adherence to treatment and drugs.
  • Monitor symptoms and side effects of treatment.
  • Delivering psychosocial care and rehabilitation, including counseling, psychotherapy and education to patients and family members.
  • Strengthen (empower) the ability of families and communities in mental health care.
  • Improve and develop partnership and good networking among agencies and with Community mental health care.
  • Strive continuously to reduce stigma and discrimination against mental health problems.
