WORK SPECIFICATION

 
Under the guidance and supervision of peer seniors, you are reasonably expected to have prime responsibility for the episode of patient care during a period of hospital admission.
 
  1. Taking and/or checking the detailed medical history and examination.
  2. Ordering and coordinating investigations.
  3. Communicating with the referring medical practitioner in writing at least once during the period of admission.
  4. Liaison with all members of the health care team to facilitate effective and efficient patient management.
  5. Daily review of all patients under your care.
  6. Documenting clinical history, physical examination and management plan in the medical record at the time of admission.
  7. Maintaining an ongoing record of the patient's progress, investigations and results.
  8. Discussing discharge plans with the patient's general practitioner and other healthcare professionals responsible for ongoing care.
  9. Writing a summary on discharge.
 

NON-CLINICAL SKILLS

 
Communication Skills
 
  1. Your communication skills are particularly important because of your central role in coordinating patient management.
  2. At all times you should be kind and courteous to patients and their relatives and families. Any complaint of discourtesy or harassment would be viewed seriously and is liable for disciplinary action. You should keep the patient informed of his condition and answer all his queries.
 
Presentation Skills
 
  1. During your internship you should have the opportunity to practice and improve your presentation skills. You will probably be expected to present patients case histories and clinical details at their bedside during regular ward  rounds.
 
Ethical Skills
 
  1. During your internship you should have the opportunity to be aware and identify medico-legal problems and avoiding unethical practices.
  2. A procedures such as consent, prescription writing, death certification, incident reports and other medic legal issues should always be considered before giving advice or carrying out any procedures.

DRESS CODE

 
At all times HO should dressed neatly in keeping with the dignity of the medical profession:
  1. Should not be unsightly such as short pants or exposing bodily parts with tight shirts or pants
  2. No t-shirt or jeans or short pants during office time
  3. No clothes showing any political affiliation
  4. Male HO must wear tie (not bow-tie) during office hours
  5. No slippers/sandals/sport shoes 
  6. No dyeing of hairs, no long hairs and the hair cut is the accepted socially (punk cut or similar cut is not permitted)
  7. No heavy make-up or jewelries is permitted.

CONTINUOUS MEDICAL EDUCATION(CME)

 
  1. It is a wrong attitude that coming to orthopaedic posting is to take a breather in between the other postings. Even if you are not interested you must acquire basic and sufficient knowledge and skills during the postings.
  2. HO must attend 3 teaching sessions:
    • Monday Morning (8.00-9.30am)
    • Tuesday Morning (8.00-9.30am=HO teaching)
    • Tuesday Afternoon( 3.00-4.00pm)=Trauma audit
  3. HO also must attend unit teaching. Tuesday morning teaching is a protected time where HO is absolved from all other activities. 
  4. Attendance to be taken in all sessions and the attendant sheet to be submitted together with the log book.
  5. HO is also assigned to do presentation during HO teaching.
  6. All new HO must do 5 consecutive days tagging started from 8.00am till 10.30 am & must follow first call HO.
 

ATTITUDES

 
Be Responsible
 
  • Arrive to work on time, exhibit a strong work ethic and use time efficiently. Display Energy & Enthusiasm- Have the Right Attitude.
  • The most successful interns are those that display an upbeat attitude and a genuine interest or positive outlook in performing the role.
 
Be Professional
  • Dress appropriately for work.
  • Even in moments of stress, maintain a positive attitude.
 
Honest
  • Mistakes is forgivable and mend for future improvement.But dishonest and lying or cheating is not acceptable at any instances. Do not assume on facts or putting unverified findings is the folders or informing unsure facts to superiors are all indicative of dishonesty.
  • Ask questions and be critical during performing the assignment. Asked why? And try to understand.Do not just do the jobs without understanding.
 
Teamwork
  • Be considerate, accommodative and helpful to colleagues and works in tandem and do not take advantages on the expense of other colleagues. Be humble to learn from nurses and paramedics
 
Patient Care
  • Learn the best way of communication, be courteous and response to any query in a manner full of empathy. Harsh words is never be cordoned.
 
Learning while Working
  • Each of the patient is your textbook where you should read about that case for eg. Patient with cauda equina syndrome. You must read what is cauda equina syndrome.
 

ASSESSMENT

 
Assessment of the Performance
  • 1 (one) month before the end of the posting, VIVA will be done to assess the competency and any weaknesses will be made known. This done by the supervisor of the unit. The failed and weak HO will be viva again within two weeks of the end of the posting to make sure on adequate competency.
  • 3 aspect to be assessed: 
  1. Knowledge: From viva
  2. Skill: From Logbook (To be submitted one month before the end of posting)
  3. Attitudes: From the supervisor
 
Viva
  • The knowledge will be assessed with regard to the learning objectives. If the outcome of the viva is not satisfactory, the HO is given the second viva. If the outcome is still unsatisfactory, the HO will be extended for 2 months
 

EXTENSION OF THE POSTING

 
The posting can be extended in the following conditions:
 
Issues in Competency
  1. Fail Viva.
  2. Fail logbook or attendance. Fail to attend the CME 3x without acceptable reasons is justified for extension.
  3. Fail attitude including repeated mistakes and blunder despite warnings given.
  4. Other Issues:
  • Leave Issue: Taking leave more than the admissible or any interferences to the training for whatever reasons.
  • Other issues such as lackadaisical attitude and other non-professional attitude despite various warning.
  • SERIOUS NEGLIGENCE such as negligence in checking blood and blood transfusion procedures  or mixing specimen deems straight extension.
