Short synopsis:

This is a presentation of a scuba diving mishap. A young experienced scuba diver ascend to 12 meters when she suddenly noted to be panicky and seems like out-of-air.

Upon surfacing she was breathless and later developed respiratory failure.

This is not a case of decompression illness as one might mistakenly diagnose as her problem started during descend as opposed to during ascend.

We postulate that the possible reason for the incident is scuba equipment failure.
