 1.  Pre-examination 1) Ensure that the percentage of rejection of test orders is no more than 0.5% of the    total number of samples registered online at the Endocrine Laboratory every month
 2.  Examintion

2) Ensuring the coefficient of variation (cv) of HbA1c for Internal Quality Control every month is less than 2.5% (running cv).

Post Examnination

3) Ensuring the reporting of test results within 8 working hours for HbA1c is not less than 95% of all test applications every month.

Sample Disposal

All 'biohazard bags' used in the delivery process should be considered as 'potentially hazardous' and disposed of in 'biohazard waste'. 'Biohazard waste' will be collected daily according to the procedures in the Quality Procedure 'Storage of Specimen After Testing Until Disposal' (HUSM/ENDO/QP-05).

Specimen Delivery Procedure

To maintain specimen integrity and staff safety, the following specimen delivery measures must be taken into account:
 
a) All specimens must be shipped in a plastic bag of biohazard specimens. Specimens and order forms should be segregated in different pockets to prevent the form from being contaminated in the event of any leakage.
 
b) Immediately send the specimen to the laboratory to avoid damage to the specimen and the accuracy of the test results.
 
c) The specimen is submitted at the Endocrine Laboratory reception counter. Confirm the specimen delivery time on the order form using the time recorder provided at the counter
